Two things to know before you enter
The prize is a prescription medicine. A licensed clinician has to decide it is appropriate for you, and may decide it is not. Winning the draw does not mean you will be prescribed anything, and nobody — not us and not the sponsor — can promise otherwise. If a clinician declines, the alternative in section 5 applies.

1. Sponsor and promoter
The promotion is administered by ED Treatment Spot (ED Treatment Spot, 123 Example Street, Suite 400, Austin, TX 78701). The prize is supplied by CoreAge Rx. CoreAge Rx is not otherwise responsible for the administration of this promotion.
2. Eligibility
- Open to legal residents of the United States who are 18 years of age or older at the time of entry, except where prohibited.

- Employees, contractors and immediate family members of ED Treatment Spot and CoreAge Rx, and of their respective affiliates, are not eligible.
- Anyone for whom this class of medication is contraindicated should not enter with the expectation of using the prize. See section 5.
3. Promotion period

4. How to enter
No purchase necessary. A purchase will not improve your chance of winning.
- Online: submit a valid email address through any giveaway form on https://www.edtreatmentspot.com. First and last name are optional. Limit one entry per person per email address.
- By post (free entry method): hand-print your name, address, email address and date of birth on a card and post it to ED Treatment Spot, 123 Example Street, Suite 400, Austin, TX 78701. Postal entries must be received before the closing date and carry the same chance of winning as online entries. Limit one per envelope.

5. Prize
One (1) prize is available: a subscription to CoreAge Rx 4PLAY for as long as the winner continues to be prescribed it, supplied by CoreAge Rx. Approximate retail value $1,368 per year, based on the sponsor’s published rate of $114 a month at the time of writing. Total value depends on how long the subscription runs and cannot be determined in advance.
The prize is a prescription medicine.The winner must complete the sponsor’s medical intake, and a licensed clinician must independently determine that the treatment is appropriate. Neither ED Treatment Spot nor CoreAge Rx can direct or override that decision.
If a clinician declines to prescribe, or if the winner is unable to take the medication for any medical reason, the winner will instead receive [CASH OR EQUIVALENT ALTERNATIVE — TO BE SET BEFORE LAUNCH] of comparable value. No medication will be supplied to anyone a clinician has not approved.
The prize is not transferable and cannot be exchanged, except that the Sponsor may substitute a prize of equal or greater value if the stated prize becomes unavailable.
6. Odds of winning
Odds depend on the number of eligible entries received during the promotion period. One winner will be selected in a random draw from all eligible entries, conducted within [NUMBER] days of the closing date.
7. Winner notification
The winner will be notified by email at the address entered, within [NUMBER] days of the draw. If the winner does not respond within [NUMBER] days, or if the notification is returned undeliverable, the prize may be forfeited and an alternate winner drawn. The winner may be required to complete an affidavit of eligibility and a liability release before the prize is awarded, where lawful.
8. Taxes
The winner is responsible for all federal, state and local taxes arising from the prize. Where the value requires it, a Form 1099-MISC may be issued and tax identification details requested.
9. Privacy
Entry details are used to administer the promotion and to send the emails you signed up for. We do not sell personal information. 10. General conditions
The Sponsor may cancel, suspend or amend the promotion if fraud, a technical failure, or any other cause beyond its reasonable control corrupts the administration, security or fairness of it. Entries that are automated, duplicated or submitted in bad faith are void.
