It is also not the cheapest option here — at about $23 a dose it sits near the bottom of the ranking below, where its price puts it. We are not going to reorder a price list to flatter a pick, so here is the case for spending more, made once and made properly.
What the money buys is the things the cheap end of this market does not sell you: the fullest safety and compounded-drug disclosure of any combination product we have audited, a formulation compounded to your own prescription, opt-in rather than automatic refills — nobody else here does that — and a refund if a clinician decides the treatment is not appropriate. Six of these publish no price at all. The other six publish a price without saying how many doses it buys, which makes a per-dose figure a guess rather than a calculation. We would rather leave them out of the ranking than invent a number and sort on it.

Four things a cheap price often leaves out
1. The second month
A headline figure is frequently a first-order discount. Ro Sparks advertises $28 and renews at $48. Keeps puts a footnote on its page saying whatever number you are looking at is a first-order price. If you are budgeting, budget on the renewal. We ranked the services on that figure separately.
2. What is not medication
Hone Health’s $50 a month is $25 of medication and $25 of mandatory membership, on top of a $65 lab test before anything is prescribed. CareFirst adds $19.95 of refrigerated shipping to every $52.95 order. Sesame’s tablets are the cheapest here, but you buy a $34 video visit first.
3. How many doses are in the box
This is where the real distortion happens. Two services on this site both charge $114 a month. One of them is five doses. A monthly price tells you almost nothing until you know what it divides by — which is the entire reason this page ranks on cost per dose instead.
4. Whether anyone told you the risks
The two cheapest services on this page sit at opposite ends of that question. The first puts you in front of a doctor on video who can actually ask. The second is a quiz and a price list, on a page that says nothing at all about nitrates, priapism or heart disease. Both are cheap. Only one of them is a bargain.
The cheapest route of all is usually not on this list
Generic sildenafil and tadalafil have been off patent for years and cost a few dollars a tablet from an ordinary pharmacy with an ordinary prescription. Every compounded blend on this page is a multiple of that, and the multiple buys convenience and a formulation — not a stronger drug. Which generic suits which situation.
Cheap is the wrong thing to optimise alone
Tell whoever prescribes this about every heart medication you take. ED pills and nitrates together can drop your blood pressure to a dangerous level. Several of the cheapest pages on this list never mention that, and a form that does not ask is not a medical intake.
A low price is not a low risk. For a man over 50, ED is often the first sign of vascular disease. No online service can take your blood pressure — so see someone in person as well, whatever you end up buying.
