Every other entry in this comparison is a telehealth funnel: answer a questionnaire, get approved, receive medication. Empower is a compounding pharmacy. There is no consultation, no plan and no questionnaire.

You need a prescription from your own licensed provider first. Then you submit it through their portal and they fill it. The page says as much: they accept prescriptions “written within their scope of practice and with a valid patient-practitioner relationship”, and directs patients to find their own provider.

We have included it because it shows what this category looks like when the clinical decision genuinely sits with a doctor who knows you — and because the documentation is a benchmark the direct-to-consumer pages should be measured against.

The dose deserves your attention

Tadalafil 40 mg with apomorphine 6 mg. The dose is in the product name, which is refreshing after four services that would not state one at all.

It is also the highest tadalafil dose in this entire comparison — double the 20 mg maximum approved for as-needed use, and eight times the 5 mg approved for daily use. For scale, one competitor's flagship contains 11 mg and another 20 mg.

That is not a scandal in this context, and the distinction matters: this is a prescriber-facing product where a clinician who has examined you chooses whether 40 mg is appropriate. It would be alarming on a page selling directly to consumers with a two-minute questionnaire. Here it is a clinical decision being made by someone qualified to make it.

The documentation is the best we have seen

Everything the direct-to-consumer pages leave out is here, and stated more precisely than any of them manage.

The nitrate interaction is a contraindication, not a caution, with nitroglycerin and isosorbide dinitrate named. The cardiac exclusions carry actual numbers — myocardial infarction or stroke within 90 days, systolic blood pressure under 90 mmHg. Side effects are separated by ingredient, and apomorphine's orthostatic hypotension is named, which no other page does. NAION and sudden sensorineural hearing loss both appear.

It also cites the statute it compounds under — section 503A — and explains what that means, alongside the flat statement that safety and efficacy for the formulation have not been evaluated by the FDA. There are eighteen references to peer-reviewed literature.

The most honest sentence in this whole comparison

On whether the combination works better than one drug alone, the page says it improves scores compared with monotherapy — and then adds: “though high-quality trials remain limited.”

Every other service selling a combination product asserts superiority over generics. One claims three times the strength. This one tells you the evidence is thin.

That single clause is the difference between a pharmacy documenting a product and a brand selling one.

What is missing

No price. Nowhere on the page. You will not know what a fill costs until you are in the portal, which is a real gap even for a prescriber-led product.

No physical address, and the “licensed in all 50 states” claim carries an asterisk noting that licensing is subject to change. For a compounding pharmacy — where the facility and its inspection record are the product — an address and licence number should be easy to find.

The bottom line

The right way to use this page is as a reference, not a shop. If you are considering a compounded combination, read this one and then take its contraindication list to your own doctor. It is the standard the rest of this category should be held to — and a reminder that the model where a real prescriber decides is still the better one.
