ED medication side effects: what is normal and what is not

Most side effects from ED medication are mild, predictable and a direct consequence of how the drug works — it opens blood vessels, and not only the ones you had in mind. A small number are genuine emergencies. Knowing which is which prevents both unnecessary alarm and the far more dangerous mistake of waiting out something that needed same-day attention.

The short version

  • Headache, flushing and a blocked nose are the most common effects, and they come from the same vessel-opening action that produces the erection.

Why these particular effects happen

PDE5 inhibitors do not act only on the penis. The enzyme they block is present in blood vessels throughout the body, so every common side effect is the same drug action showing up somewhere you did not want it.

That is worth understanding for two reasons. It means the common effects are a sign the drug is doing what it does rather than a sign something is wrong. And it means a tablet that produces no sensation whatsoever is slightly suspicious — a genuine dose usually announces itself in some small way.

Common and usually harmless

HeadacheThe most common of all. Plain paracetamol is fine alongside. Drink water; it is worse when dehydrated. Often milder on a lower dose.
Facial flushing, feeling warmHarmless and short-lived. Alcohol makes it noticeably worse.
Blocked or runny noseVessels in the nasal lining dilating. An ordinary decongestant helps; check with a pharmacist if you take blood pressure medication.
Indigestion, refluxMore common with sildenafil. Avoid taking it on a very full stomach or late at night.
Back or muscle acheMore associated with tadalafil, typically starting 12 to 24 hours after the dose and lasting a day or two. Usually settles with continued use, and responds to ordinary painkillers.
DizzinessStand up slowly. If it is marked, mention it — it can indicate the dose is too high or is interacting with a blood pressure drug.

These generally ease over the first several doses. Men who quit after one uncomfortable experience often would have been fine by the third, which is worth knowing before writing the drug off. When a fair trial is and is not fair.

The visual thing sildenafil does

Sildenafil has a mild effect on an enzyme in the retina, and at higher doses some men notice a blue tinge to their vision, increased brightness sensitivity, or slightly blurred edges.

This is temporary, well documented, and not the same thing as the emergency described below. It resolves as the drug wears off. If it bothers you, tadalafil does not share the effect to the same degree and is the obvious alternative. How the two compare.

Stop and get help the same day

These are rare. They are also the ones where hours matter, and where men lose outcomes by waiting until morning or feeling too embarrassed to attend.

The vision one has a name — non-arteritic anterior ischaemic optic neuropathy — and it is very rare. Whether these drugs cause it or simply appear alongside the same vascular risk factors is still debated. Either way the response is identical: stop taking it and be seen immediately, because the window for treating it is short.

What makes side effects worse

  • A dose higher than you need. Many men are on 100 mg of sildenafil because it was escalated once and never revisited. If it works reliably, it is reasonable to ask whether 50 mg would too.
  • Alcohol. Amplifies flushing, headache and dizziness, and lowers blood pressure alongside a drug already doing so.
  • Dehydration, which makes the headache considerably worse.
  • Blood pressure medication, particularly alpha-blockers — not a reason to avoid ED drugs, but a reason for the prescriber to see the whole list.
  • Grapefruit juice, which slows the breakdown of these drugs and effectively raises the dose you took.

Daily dosing changes the pattern

Low-dose daily tadalafil produces a different side effect profile from an on-demand dose: lower peaks, so less pronounced headache and flushing, but a low-level ache that some men notice continuously rather than in bursts.

For men who find the on-demand experience unpleasant but want the medication, this is a genuine alternative rather than simply a smaller dose. It is worth raising specifically. More on the oral options.
