Why “dissolves in your mouth” is not the same as “works faster”
A large share of the online market sells troches, chews and rapid-dissolve tablets on the promise of speed, with the reasoning that absorbing a drug through the lining of the mouth skips the digestive tract and the liver.
The theory is sound for some drugs. Whether it delivers for these ones is a separate question, and the best available check comes from the one product in this format that has been through an approval.
Compounded troches making the same claim have no published pharmacokinetics at all. If the approved product in the category does not beat a tablet, a compounded one asserting that it does is asking you to take its word for something the regulated version could not demonstrate.
There is a real benefit to these formats. It is not needing water and not having to swallow. That is worth paying for if it applies to you. What ED medication should cost.
What genuinely is fast
Injections — five to fifteen minutes
The fastest and most reliable option there is, because the drug goes straight into the erectile tissue and does not depend on a nerve signal arriving or an artery responding. It works for the large majority of men, including most who get nothing from pills.
The needle is the objection, and it is almost always anticipatory — it is very fine and goes into tissue with few pain receptors. The first dose is given in clinic so the response can be watched. What injections involve.
The topical gel — about ten minutes
The over-the-counter gel works in around ten minutes, which is faster than any pill. It is drug-free, so it also carries no nitrate interaction. The trade is that it works for fewer men — the manufacturer's figure is over 60% — and mainly in mild to moderate ED. Read the review.
Devices — immediately
A vacuum pump takes a couple of minutes of pumping. A constriction ring works the instant it is on. Neither depends on chemistry, which is why they keep working when everything else has stopped. How vacuum devices work.
The two things that slow a pill down
A heavy meal. Sildenafil absorption is substantially delayed by food, especially a fatty meal — this is the single most common reason a tablet seems not to have worked. Take it on a relatively empty stomach, or at least an hour before a large dinner rather than at the end of one. Tadalafil is essentially unaffected by food.
Alcohol. Not an absorption problem so much as a physiological one. A couple of drinks is usually fine; more works directly against the erection and against the drug. Alcohol, smoking and ED.
The question behind the question
Men ask about speed because they are trying to solve spontaneity — the problem of having to plan an hour ahead, and of a partner knowing that a tablet was taken.
Speed is one answer to that. Duration is the better one, and it is the one that gets overlooked. Tadalafil as needed lasts up to 36 hours. Take it on a Friday evening and Saturday is simply available, with no timing to manage at all.
Low-dose daily tadalafil takes the idea further: 2.5 or 5 mg every day, reaching a steady level after about a week, after which there is nothing to time and nothing to plan. For men who want sex to stop being an appointment, this solves the actual problem far better than shaving twenty minutes off an onset time. It is also usually among the cheapest options. How sildenafil and tadalafil compare.
If it is not working, check these before changing product
- Did you wait long enough? An hour for sildenafil, and longer after a big meal.
- Was there enough stimulation? None of these drugs create desire. They open a door that arousal still has to walk through.
- Have you tried it enough times? Eight attempts is the standard threshold before calling a drug a failure. Most reported failures are not.
- Are you on the right dose? Many men are started at the maximum by services that only sell one strength, or left on a starting dose that was never reviewed.
- Is something else in the way? Alcohol, a heavy meal, exhaustion, or a medication you take for something else. Medications that cause ED.
