Shameless Duo review

81 mg of sildenafil and 12 mg of tadalafil in one troche, and it tells you so

Best for: Nobody starting out — the doses are published, and they are the reason to be careful← All reviews

Our verdict

One of the more honest compounded pages we have audited — it publishes both doses, gives the beyond-use date, and does not pretend the FDA has looked at it. Then it sells 81 mg of sildenafil and 12 mg of tadalafil in a single troche and calls the size of that dose the feature. Because the numbers are public we can be precise about the objection instead of vague, which is more than most of this market allows.

Safety checks

The same seven checks for every service on this site, all read off its public page. The first matters most: ED medication taken with nitrates can cause a life-threatening drop in blood pressure, so a page that never mentions it is leaving out the one thing that could kill you.

  • Discloses the nitrate interaction: No

No nitrate warning on the product page. For a compounded product containing two vasodilators at close to their individual maximum doses, this is the most serious omission possible.

  • Warns about erections lasting over four hours: No

No four-hour rule on the audited page, despite a combined dose that raises the risk of a prolonged erection above either drug alone.

  • Lists side effects on the page: No

No side effect list on the product page.

  • Raises heart disease and blood pressure: No

Nothing about heart disease, blood pressure, or existing cardiac medication.

  • States it is compounded and not FDA-approved: Yes

Stated twice and stated correctly: compounded medications are not FDA approved, and FDA approval applies to commercially manufactured drugs rather than individually compounded prescriptions. That second sentence is more accurate than most of this market manages.

  • Publishes the actual doses: Yes

Sildenafil 81 mg and tadalafil 12 mg, stated on the page, in the product description and again in the FAQ. Also publishes a five-month beyond-use date.

  • States cancellation terms before you buy: Partly

Sold as one-off packs of 12 or 24 rather than an auto-renewing subscription, which sidesteps most of the problem, but no terms are stated on the page.

Good fit if

  • Men who have already used both drugs separately, at full dose, under supervision
  • Anyone who specifically wants the dose written down before buying a compounded product

Look elsewhere if

  • You take nitrates in any form — this page will not tell you, so we will
  • You have not tried a single ED drug at a proper dose yet
  • You have heart disease, uncontrolled blood pressure, or take an alpha-blocker
  • You want value — this is five times the price of an FDA-approved generic

What you are actually buying

A compounded sublingual troche containing sildenafil 81 mg and tadalafil 12 mg, dissolved under the tongue. $130 for twelve or $170 for twenty-four — about $10.83 and $7.08 a dose.

Credit where it is due. The doses are on the page, in the description and again in the FAQ. The page states twice that compounded medications are not FDA approved, and adds the more precise point that approval applies to commercially manufactured drugs rather than individually compounded prescriptions. It even publishes a five-month beyond-use date. Most of the compounded market does none of this.

Now the arithmetic those numbers allow

Because the doses are public, the concern is measurable rather than rhetorical.

DrugIn this trocheMaximum as-needed dose
Sildenafil81 mg100 mg
Tadalafil12 mg20 mg

Individually neither number is over a limit. Together, in one dose, they add up to more total PDE5 inhibition than either drug is approved to deliver on its own — and there is no approved product, and no guideline, that combines two drugs from this class.

The page's own words are that it delivers “a far more powerful dose than standard generics”. That is accurate. It is being offered as the reason to buy, and it is the reason to be careful. Why so much of this market stacks.

What the page does not say

No nitrates. No four-hour rule. No side effects. Nothing about heart disease, blood pressure, or the alpha-blockers commonly prescribed for an enlarged prostate.

That combination — precise doses, honest compounding disclosure, and no clinical warnings whatsoever — is a specific and quite common failure. Somebody here cared about being accurate. Nobody wrote the safety section. What a missing warning tells you about a product page.

The money

Between $7 and $11 a dose, depending on pack size. Generic sildenafil from an FDA-approved supplier runs about $2 a pill. You are paying roughly four to five times as much for a compounded combination with no approval behind it and no trial demonstrating that the combination beats either drug alone.

The bottom line

The most transparent page in the compounded troche category, selling the product we would be slowest to recommend. If you have used both drugs separately at full dose and discussed combining them with a prescriber who knows your heart history, at least here you know exactly what is in it.

For everyone else: one drug, a starting dose, a label in the box, and a doctor who has taken your blood pressure. How the two drugs differ and which to try first.

What is good

  • Publishes both doses precisely — 81 mg sildenafil and 12 mg tadalafil
  • States clearly, and more than once, that compounded medications are not and cannot be FDA approved
  • Gives the beyond-use date, which almost no compounded seller does
  • Sold as packs rather than an auto-renewing subscription
  • Explains the sublingual route and why it claims faster absorption

What is not

  • Two PDE5 inhibitors in one dose, both near their individual maximums
  • No nitrate warning on the page, for a product containing two vasodilators
  • No four-hour rule and no cardiac screening language
  • About $7 to $11 a dose, against $1.99 for an FDA-approved generic
  • Markets the high dose as the selling point — “far more powerful than standard generics”
