Sesame ED consultation review

It is a marketplace, not a brand

Every other entry in this comparison sells you its own product. Sesame sells you access to a clinician. You browse providers, see each one's rating and fee up front, book a video visit of about fifteen minutes, and they send a prescription to your pharmacy or arrange delivery.

That is a meaningfully different proposition. A questionnaire can only ask what it was written to ask. A conversation lets you mention that you still wake with erections, or that this started three weeks after a new blood pressure tablet — the details that actually narrow the diagnosis and that no form thinks to request. Why that detail matters.

It also means the person deciding is a named clinician with a visible rating, not an anonymous reviewer working through a queue.

The pricing is the most honest here

Sesame publishes what everything costs, itemised, before you commit:

Tadalafil at $18.50 for thirty tablets works out at about 62 cents a dose — the cheapest medication in this entire comparison by a wide margin. For contrast, two compounded services in our set charge $114 a month, and one works out at $11.50 a dose.

And there is no subscription. Nothing auto-renews, nothing needs cancelling. In a category where billing complaints outnumber safety complaints, that removes the most common problem entirely.

Note the avanafil price, though: around $175 for five tablets. That is the one genuinely expensive item on the list, because generic avanafil supply is still limited. If price matters, sildenafil or tadalafil is the conversation to have.

It sells approved medicine

All four PDE5 inhibitors, as FDA-approved generics: sildenafil, tadalafil, vardenafil and avanafil. No compounded combinations, no stacked drugs, no peptides.

That is worth stating plainly after nine reviews of compounded products. Here the medicine has been through trials and approval, the tablet contains what the label says, and a regulator has verified it. Why that distinction matters.

The nitrate warning is good. The rest is not.

On nitrates, Sesame does the thing we have been asking for throughout this comparison — it names the interaction *and* the consequence:

“Combining them can cause dangerously low blood pressure, which can lead to fainting, heart attack, or stroke.”

Then it stops. There is no four-hour erection warning. There is no mention of sudden vision or hearing loss. Side effects appear once, in passing, as “a headache, sensitivity to light, facial flushing or an upset stomach, although these are usually temporary” — the reassuring framing rather than the label.

There is a partial defence: on this model you will be talking to a clinician who is supposed to cover exactly this, so the page is not the last line. But it is the page where the decision to book gets made, and men who take nitrates need to know before they pay.

One thing it gets right that almost nobody does

Buried in its explanation of causes, Sesame says that erectile dysfunction “may also be a sign of an underlying health problem or a risk factor for heart disease”, and lists poor vascular health among the causes.

That is the single most useful thing a man over 50 can learn about this symptom, and it appears on almost none of the commercial pages we have read. The bottom line

For a man who wants a real conversation, approved medicine, no subscription and the lowest price available, this is the strongest option in our comparison. Go in knowing the two things the page will not tell you: mention every heart medication you take during the visit, and treat an erection lasting over four hours as an emergency.
